Description : 1. The Municipal Government of Benito Soliven, Isabela through its Bids and Awards Committee, will undertake Negotiated Procurement Two Failed Bidding in accordance with section 35 of the Implementing Rules and Regulations of Republic Act No. 12009 or the New Government Procurement Act.

2. The Municipal Government of Benito Soliven, Isabela hereinafter referred to as End- User requests interested applicants to submit quotation.

3. Interested suppliers are required to submit their valid and current Mayor's Business Permit, PhilGeps registration Number, Omnibus Sworn Statement and Income /Business Tax return, if applicable, in a sealed envelope marked: (Envelope 1- Name of the Project- eligibility Requirements).

4. All request/s for Quotations (RFQs0 must be delivered in hard copies placed in a sealed envelope marked (Envelope 2- Name of Project- Price Quotation). All Entries must be typewritten or handwritten eligibly.

5. Quotations must be delivered at the address below not later than 10:00 AM of December 22, 2025:
