Description : INFUENZA VACCINE TERMS OF REFERNCE.
I. SERVICES AND ADMINSTRATION:
Vendor/Supplier-Shall provide medical team for the on-site vaccination both DAP Pasig and DAPCC-Tagaytay.
II. IMPLEMENTATION AND DURATION:
2 DAYS FOR Pasig and 1 day for DAPCC-Tagaytay.
Personnel of DAP sa Mindanao will have different arrangement
III. DEPLOYMENT OF MEDICAL TEAM:
Minimum of 200 vials to be administered per day with 1 MD and 2 RNs for 9 straight hours with 1-hour lunch break. The clinic shall oversee the program.
IV. PACKAGING:
Styropor and Ice Gels care of the Vendor/Supplier.
V. FORMS AND MEDICAL SUPPLIES:
- Vendor/Supplier shall provide Screening forms and Vaccination Cards.
- Vendor/Supplier shall provide Alcohol prep pads, Band Aids, and Sharps Container.
VI. PICK UP AND COLLECTION:
Vendor/Supplier- Shall Pick up materials used exclusively during the vaccination such as:
1. Styrofoam boxes
2. Ice gels
3. Accomplished Forms
4. Medical Materials and
5. Sharp Containers
