b. Certified Photocopy of the MAYOR'S and SANITARY PERMIT ISSUED BY THE CITY OR MUNICIPALITY WHERE THE PRINCIPAL PLACE OF BUSINESS OF THE PROSPECTIVE BIDDER IS LOCATED.
c. Certified Photocopy of DTI Certificate, Tax Clearance, BIR(Certificate of Registration), and Audited Financial Statement.
d. Original Copy of Omnibus Sworn Statement
2. For Red Registration:
a. Certificate of PhilGEPS Registration b. Mayor-s/Business (CY 2023)
