Description : PROCUREMENT OF SERVICE PROVIDER FOR THE TRANSPORT AND TREATMENT OF HAZARDOUS WASTES UNDER ILD-RSL
SCOPE OF WORK:
1. Hauling, treatment and disposing of the following hazardous wastes:
a) Containers previously containing toxic chemicals (Class code J201), 50kg;
b) Pathogenic or infectious wastes, sharps and others (Class code M501), 75kg; and
c) Chromium compounds with concentration 5 mg/L based on analysis extract (Class code D405), 1800 liters;
2. The service provider must be an accredited DENR-EMB Transporter of hazardous wastes, with valid Transportation Registration Certificate;
3. The service provider must secure from EMB XI the Manifest Form prior to hauling;
4. The service provider will provide the end-user a notarized “Certificate of Treatment”; and
5. The service provider will assume full responsibility for any accident or damage that may arise during transport of the hazardous waste.
