1. Chest X-Ray
2. Laboratory Examination consisting of the following :
a) Complete blood count (CBC)
b) Urinalysis
c) ECG
d) Blood Chemistry
- Fasting Blood Sugar (FBS)
- Total cholesterol
- Triglycerides
- HDL (good cholesterol)
- LDL (bad cholesterol)
- VLDL
- Uric acid
- Blood Urea Nitrogen (BUN)
- Creatinine
- SGPT
e) HBAIC
3. Flu Vaccine with expiratiion date not earlier than six (6) months from
reciept of Purchase Order
4. Abdominal x-ray
- kidney, uterus, bladder, liver, pancreas, stomach, intestine, spleen
NOTE:
a) Blood extraction, submission of urine specimens, ECG, X-ray
and administration of flu vaccines shall be conducted on-site.
b) Mobile Clinic for X-ray shall undergo disinfection spraying
at least every thirty (30) minutes for the whole duration of on-site
APME as safety protocol
c) Clean laboratory gowns (for X-ray) and bed/bedding (for ECG)
