Terms of Reference:
1. The Contract shall be valid for One (1) year from the date of receipt of Purchase Order
2. The Provision of Services shall be on a withdrawal basis or upon request of the end-user.
3. The interested suppliers shall submit Environmental Sanitation Clearance issued by the Department of Health as part of the eligibility requirements.
4. Perform basic inspection of the septic system during each service call to identify potential issues.
5. Safely transport and dispose of waste to accredited and authorized disposal facilities in compliance with environmental laws.
6. Submit a Waste Disposal Certificate after each service indicating date, volume collected, and disposal site.
7. The supplier shall issue to the end-user a copy of Manifest Form signed by the TSD facility.
8. Payment shall be made after the supplier issues a Billing Statement, Service Report, and Disposal Certificate,
