3. WARRANTY SHALL BE FOR A PERIOD OF SIX (6) MONTHS FOR SUPPLIES & MATERIALS, ONE (1) YEAR FOR EQUIPMENT, FROM DATE OF ACCEPTANCE BY THE PROCURING ENTITY
4. PRICE VALIDITY SHALL BE FOR A PERIOD OF NINETY (90) CALENDAR DAYS

-LATEST INCOME TAX RETURN

6. ALL SUBMITTED DOCUMENTS MUST BE CERTIFIED TRUE COPIES OF THE ORIGINAL DOCUMENTS, BEARING A SIGNATURE ON EVERY PAGE.
7. SPECIAL POWER OF ATTORNEY (SPA) AND IDENTIFICATION OF THE OWNER AND ITS AUTHORIZED REPRESENTATIVE.
